Appointment Request

Please complete the form below, and a member of our veterinary team will get back to you as quickly as possible to confirm your appointment. Please note: This is just a request. An appointment will be confirmed after we contact you.

New client appointments are scheduled on Tuesdays, Thursdays, and Fridays. Appointment options will be provided in a follow-up email after this form is submitted.

Our Fees

Housecall fee: $99 (charged once per visit)
Consultation fee: $66 per pet
Vaccines: $39 each
Additional fees are available on request. A complete estimate is created at the time of the visit, tailored to your pet’s needs. There may be an additional travel fee for locations outside our primary service area of Laveen, AZ.
